29 CFR 1910.147(c)(7)(i)(A):Authorized employee(s) did not receive training in the recognition of applicable hazardous energy sources, the type and magnitude of the energy available in the workplace, and the methods and means necessary for energy isolation and control: Nova Tube and Steel, LLC located at 8641 County Road H in Delta, Ohio 43515: On or about July 24, 2025, the employer did not ensure that employees performing lockout covered activities on energized equipment such as but not limited to, the OTO and MTM lines were provided with adequate lock out tag out training. Employees performing the tasks such as, but not limited to, blade changes, changeovers, housekeeping, and clearing jams, were exposed to amputation, struck by, and caught in hazards associated with spinning blades, left and right movement of blade carriages, and/or stored tension on jammed steel tube.
