29 CFR 1926.1204(i): (i) Develop and implement procedures for summoning rescue and emergency services (including procedures for summoning emergency assistance in the event of a failed non-entry rescue), for rescuing entrants from permit spaces, for providing necessary emergency services to rescued employees, and for preventing unauthorized personnel from attempting a rescue; a) On or about October 30, 2024, at the AAC gas pit at the Cincinnati Zoo, John E. Carrigan Plumbing Co. failed to develop and implement procedures for summoning rescue and emergency services, including procedures for non-entry rescue, providing emergency assistance to rescued employees, and preventing unauthorized personnel from attempting a rescue. As a result, employees entered a permit-required confined space without established procedures for responding to an emergency, including how to summon rescue services or conduct a non-entry rescue if needed.
