29 CFR 1915.12(d)(1): The employer did not ensure that each employee entering a confined or enclosed space or other areas with dangerous atmospheres was trained to perform all required duties safely. On or about, March 15, 2024, on board the Cuyahoga vessel moored at the Port of Ashtabula, Kinder Morgan Pinney Dock; Minnesota slip, A/B dock side, employees entered confined spaces in the water ballast tanks and dangerous atmospheres in cargo hold #2 to perform hot work activities without being trained by the employer on how to safely perform duties while working in permit required spaces.
