29 CFR 1926.451(f)(7):Scaffolds were not erected, moved, dismantled, or altered, by trained and experienced employees under the supervision and direction of a competent person qualified in scaffold erection, moving, dismantling or alteration:; Such activities were not performed only by experienced and trained employees selected for such work by the competent person. Location: Worksite On or about January 31, 2024 employees were exposed to fall hazards of 25 feet where the employer did not ensure that a competent person qualified in scaffold erection built the scaffolding being utilized on site. The employer has previously failed to ensure that scaffolds were erected, moved, dismantled, or altered, by trained and experienced employees under the supervision and direction of a competent person qualified in scaffold erection, moving, dismantling or alteration, and that such activities be performed only by experienced and trained employees selected for such work by the competent person, as observed by CSHO on January 31, 2024, and for the prior 451(f)(7), or equivalent, serious violations of this standard by this employer: a.) OSHA previously issued three citations alleging Serious violations of this occupational safety and health standard, or equivalent standards, 29 CFR 1926.451(a)(1), 29 CFR 1926.451(g)(1), and 29 CFR 1926.454(a), which was contained in OSHA inspection number 1370196, citation numbers 1 item number 1, citation number 1 item number 2a, and citation number 1 item number 2b, and was affirmed as a final order date on 2/19/2019, with respect to a workplace located at 20 Village Hill Rd., Belmont, MA 02478 where an employee fell more than 16 feet from a roof bracket scaffold system down to the driveway below.
