29 CFR 1910.28(b)(3)(ii): The employer did not ensure that each employee be protected from tripping into or stepping into or through any hole that is less than 4 feet (1.2 m) above a lower level by covers or guardrail systems. (a) 3008 Penns Valley Road, Centre Hall, PA; the Grit Screening Room - On or about October 30, 2023, the floor had 14 inch wide by 39-inch-long floor grate for water control that had sunken and recessed 4 inches below the walking working surface exposing employees to a tripping hazard. (b) 3008 Penns Valley Road, Centre Hall, PA; Crop Storage Fluming - On or about October 30, 2023, employee(s) working in the area of the fluming conveyor were not protected from falls onto the conveyor. Abatement certification required within 10 days after abatement date. The certification shall include a statement that abatement is complete, date and method of abatement, and states employees and their representatives were informed of this abatement.
