29 CFR 1910.178(o)(1): The employer did not ensure that only stable or safely arranged loads were handled. Caution was not exercised when handling off-center loads which cannot be centered. a. On or about September 11, 2023, the employer failed to ensure that only stable loads were handled by forklift operators. Affected employee(s) were exposed to crush hazards while using the Komatsu 45 LP forklift to pull bundles of 12 quartz slabs from a closed-top shipping container in a manner that caused the bundles to become unstable, thereby exposing employees to struck-by and crushing injuries.
