29 CFR 1910.146(d)(3)(vi): The employer did not verify that conditions in the permit space are acceptable for entry throughout the duration of an authorized entry: (a) On or about August 23, 2023, and at times prior to, employees working in dryer C were exposed to an excessive heat hazard in that no actual temperatures were taken in the work area and documented ensuring it was safe prior to and during entry.
