29 CFR 1910.28(b)(3)(iv): The employer did not provide that each employee be protected from falling into a ladderway floor hole or ladderway platform hole by a guardrail system and toeboards erected on all exposed sides, except at the entrance to the hole, where a self-closing gate or an offset must be used. a. Outbound - On or about December 20, 2022, employees were observed working on the conveyor system catwalk over the outbound loading areas, the ladder access to the catwalk did not have a self-closing gate, employees were exposed to a fall hazard. b. Pump Room - On or about December 20, 2022, employees access the top of the maintenance shop using a fixed ladder that doesn't have a self-closing gate at the top, employees are exposed to fall hazards.
