29 CFR 1910.178(o)(1):Load(s) on powered industrial truck(s) were not stable or safely arranged: a) Rumpke Waste & Recycling Services located at 431 Prospect Dr. Findlay OH 45840: On or about April 11, 2022, the employer did not ensure the load (McNeilus front load refuse trucks hydraulic lifting arm) was adequately secured and evenly distributed on the forks of the Hyster powered industrial truck. Employees operating in and around this condition were exposed to struck by, caught in, and crushing injuries from the unsecured/unbalanced load.
