29 CFR 1910.146(i)(4):The employer did not ensure that an attendant remains outside the permit space during entry operations until relieved by another attendant a) Omnisource LLC is located at 1500 Old Bowman Street Road Mansfield, OH, 44903: On or about September 9, 2021, an employee entered a permit-required confined space, Harris Baler Model TG 2102 to perform welding on a baler ram and was not provided with an attendant that remained outside the permit space until relieved by another attendant. A metal ram dropped approximately 8 inches hitting the entrant in the head and shoulder while no attendant was available to report on the entrant's status or assist with the rescue.
