Text For Citation: 01 Item/Group: 002C Hazard:
29 CFR 1910.147(d):Application of control. The established procedures for the application of energy control (the lockout or tagout procedures) shall cover the following elements and actions and shall be done in the following sequence: (2) Turn off and shut down the machines, (3) Physically locate and operate energy isolating devices in such a manner as to isolate the machine or equipment from the energy source, (4) Affix lockout or tagout devices to each energy isolating device, (5) All potentially hazardous stored or residual energy shall be relieved, disconnected, restrained, and otherwise rendered safe, (6) Prior to starting work on machines that have been locked out or tagged out, verify that isolation and deenergization of the machine or equipment have been accomplished a) On or about July 13, 2021, employees who performed cleaning duties on and around Fermentation Tank 5 were exposed to burn hazards from the Hot Liquor Tank (HLT) in that the HLT valve was not fully closed and the employer did not ensure (2) to (6) were followed before employees detached the hose.
