29 CFR 1910.147(f)(3)(ii)(D):Each authorized employee did not affix a personal lockout or tagout device to the group lockout device before working on the machine or equipment: (a) PHS Area: On or about 05/19/2021, and at times prior, the employer exposed employees to struck-by and caught-in hazards, in that the employer did not require groups of two or more employees to apply their locks to a group lock when locking out equipment such as, but not limited to, PHS 1 Line, PHS 2 Line, and PHS 4 Line.
