29 CFR 1926.502(d)(21):Personal fall arrest systems were not inspected prior to each use for wear, damage, and other deterioration, and/or defective components were not removed from service: (a) On or about April 29, 2021, an employee using a personal fall arrest system (PFAS) with a lifeline that was approximately 4 years past the manufacturer(s) life expectancy and not removed from service was working to install blocking onto a parapet wall and exposed to a fall hazard.
