29 CFR 1910.146(d)(3): The employer did not develop and implement the means, procedures, and practices necessary for safe permit space entry operations, including, but not limited to items (3)(i) through (3)(vi) of this paragraph: a) Production floor, Cutter Compactor, Erema 1: On March 24, 2021 and at times prior, the employer allowed employees to work in the confined spaces of the cutter compactor without developing and implementing the means, procedures, and practices necessary for safe permit space entry operations, exposing employees to struck-by, burned-by, and entrapment hazards. b) Production floor, Cutter Compactor, Erema 2: On March 24, 2021 and at times prior, the employer allowed employees to work in the confined spaces of the cutter compactor without developing and implementing the means, procedures, and practices necessary for safe permit space entry operations, exposing employees to struck-by, burned-by, and entrapment hazards. c) Production floor, Cutter Compactor, Erema 3: On March 24, 2021 and at times prior, the employer allowed employees to work in the confined spaces of the cutter compactor without developing and implementing the means, procedures, and practices necessary for safe permit space entry operations, exposing employees to struck-by, burned-by, and entrapment hazards.
