29 CFR 1910.147(c)(4)(ii)(D): Specific requirements for testing a machine or equipment to determine and verify the effectiveness of lockout devices, tagout devices, and other energy control measures. (a) At the facility, on or about September 3, 2020, the employer failed to test the vertical dock levelers to verify their published equipment specific lockout / tagout procedures, which would have identified the lock-out pin as a lockout / tagout point, thereby exposing employees to crushing, struck-by, and caught-in hazards.
