29 CFR 1910.272(g)(3): In the grain handling facility an observer, equipped to provide assistance, was not stationed outside the bin, silo, or tank being entered by an employee: On or about October 3, 2019, an employee entered a grain bin from the top, in order to walk down grain and increase grain flow from the grain bin, and the employer did not have an observer, equipped to provide assistance, stationed outside the grain bin, at the Keithsburg grain facility.
