On or about June 24, 2019 and at times prior thereto, an employee of Tarco of Texas was operating a Power Industrial Truck (PIT) down a ramp located outside that was wet from rain, without being required to slow down, exposing employee(s) to the hazards of being struck by and/or crushed by PIT.
