29 CFR 1910.147(c)(7)(i) (A): Each authorized employee(s) did not receive training in the recognition of applicable hazardous energy sources, the type and magnitude of the energy available in the workplace, and the methods and means necessary for energy isolation and control: a) On or about June 12, 2019 , Employees exposed to an amputation and caught-in hazard while they removed jams from chains, sprockets, greased equipment, and reaching into machines on lines such as but not limited to extrusions 1-8 and mixers 1-6 The employer did not ensure that employees were able to identify and properly lock out all energy sources.
