29 CFR 1910.147(c)(4)(ii)(D): The energy control procedures did not clearly and specifically outline the requirements for testing a machine or equipment to determine and verify the effectiveness of lockout devices, tagout devices, and other energy control measures: (a) On or about March 26, 2019 - 660 Industrial Blvd., Albertville, AL, employees were exposed to struck-by and caught-in hazards when the lockout/tagout procedures did not describe the specific procedures for determining that lockout had been effective.
