29 CFR 1910.147(d)(3): All energy isolating devices that were needed to control the energy to the machine or equipment were not physically located and operated in such a manner as to isolate the machine or equipment from the energy source(s). a) Parking Lot - Employees performed servicing and maintenance activities on vehicle, including a 1997 Kenworth T800 tractor, and the employer did not ensure the employees had utilized any means to secure the tractor from unexpected movement, which thereby exposed employees to serious injuries, on or about January 2, 2019. Abatement certification and documentation required within 10 days after abatement date. The certification shall include a statement that abatement is complete, the date and method of abatement, and state that employees and their representatives were informed of this abatement. Abatement documentation shall include documents demonstrating that abatement is complete, such as evidence of the purchase or repair of equipment, photographic or video evidence of abatement or other written records.
