29 CFR 1926.454(c)(1): The employer did not retrain each employee where changes at the worksite present a hazard about which an employee has not been previously trained. This violation occurred on December 18, 2018 at the 115 E. 5th St., Tulsa, Oklahoma worksite: Employees involved in the dismantling of a transport platform system were not trained in the additional precautions necessary when the initial tie in was higher than the maximum specified by the manufacturer exposing employees to the hazards of falls.
