29 CFR 1926.502(i)(4): All covers were not color coded or marked with the word "HOLE" or "COVER" to provide warning of the hazard: (a) Location - 98 Essex Street, Haverhill, MA (Roof): On or about 12/18/2018, sheet plastic, a sheet of plywood, and several pieces of dimensional lumber collectively covering a hole in the roof of an eight story building were not color coded or marked to provide warning. (b) Location - 98 Essex Street, Haverhill, MA (1st Floor): On or about 12/18/2018, plywood covering core holes in the 1st Floor of the building were not color coded or marked to provide warning.
