29 CFR 1910.147(c)(4)(ii)(D): The energy control procedures did not clearly and specifically outline the requirements for testing a machine or equipment to determine and verify the effectiveness of lockout devices, tagout devices, and other energy control: On or about November 26, 2018, employees perform maintenance and servicing on various equipment. Employees were exposed to struck by, caught in and amputation hazards in that: a) Curing Machine - The verification step of the lockout/tagout procedure for gas, does not specify how to verify that the two gas sources were locked out. b) Curing Machine - The verification step of the lockout/tagout procedure for water, does not specify how to verify that the two water sources had been locked out. c) Curing Machine - The verification step of the lockout/ tagout procedure for steam says "right side steam 1.2Mps" this does not specify how to verify that the steam has been locked out. d) Curing Machine - The verification step of the lockout/tagout procedure for pneumatic says "listen for release of air" this does not specify how to verify that the pneumatic energy sources has been lock out.
