29 CFR 1910.218(f)(1): Mechanical forging presses. When dies are being changed or maintenance is being performed on the press, the following shall be accomplished: 29 CFR 1910.218(f)(1)(i): The power to the press shall be locked out. a. On or about October 1, 2018, the employer did not ensure that press AMP #11 had its power locked out when performing service and maintenance activities on the presses, i.e. tool changes/change over and other set-up activities, thereby exposing the employees to caught-in hazards from the unexpected start-up and/or release of stored energy. b. On or about October 1, 2018, the employer did not ensure that press Farina #17 had its power locked out when performing service and maintenance activities on the presses, i.e. tool changes/change over and other set-up activities, thereby exposing the employees to caught-in hazards from the unexpected start-up and/or release of stored energy.
