29 CFR 1910.135(a)(2): The employer did not ensure that a protective helmet designed to reduce electrical shock hazard was worn by each affected employee when near exposed electrical conductors which could contact the head: On or around March 6, 2018 in the OMCC area of the Galveston Bay Refinery, the employer did not ensure that I&E technicians working at Load Center 46 wore protective helmet designed to reduce electrical shock hazard when near exposed electrical conductors that had been exposed when electrical panels on the motor control center for the F35A pump was open.
