29 CFR 1915.12(b)(1)(ii): The employer did not ensure that spaces and adjacent spaces that contain or have contained combustible or flammable liquids or gases were tested by a competent person to determine the concentration of flammable vapors and gases within the space prior to entry by an employee: (a) William E Strait - On or about January 19, 2018 employees entered enclosed spaces where odor of flammable gas had been detected and the spaces were not tested to determine the concentration of gas prior to entry.
