29 CFR 1910.146(g)(1): The employer did not provide training so that all employees whose work was regulated by 29 CFR 1910.146 (permit required confined spaces) acquired the understanding, knowledge, and skills necessary for the safe performance of the duties assigned under 29 CFR 1910.146: a) Mercy Health Systems, d.b.a. St Vincents Medical Center, located at 2213 Cherry Street in Toledo, Ohio; On or about November 6, 2017 the employer did not ensure each employee required to enter a permit required confined space was trained with the knowledge and skills necessary to safely perform entry of a permit required confined space, such as but not limited to, the San-I-Pak Pacific, Inc. trash compactor, a decommissioned autoclave. Employees entering (upper body) the San-I-Pak trash compactor to set pins were exposed to crush hazards from to the energized ram.
