29 CFR 1910.184(e)(3)(iii): A thorough inspection of alloy steel chain slings was not performed by a competent person designated by the employer, and did not include a thorough inspection for wear, defective welds, deformation, and increase in length. Where such defects or deterioration are present, the sling shall be immediately removed from service: (a) BOS Solutions, @ Anadarko Rig X22, 2000 CR 23, Fort Lupton, CO 80621: On or before November 28, 2016, as part of injury/accident prevention responsibilities under its safety program, the exposing employer did not ensure that thorough inspections of alloy steel chain slings were performed by a competent person designated by the employer, to include a thorough inspection for wear, defective welds, deformation, and increase in length. Where such defects or deterioration are present, the sling were not immediately removed from service. A short chain was used to lift a shale metal bin onto a ground mat, and snapped. This condition exposed employees to the hazard of being struck-by the chain and falling loads.
