29 CFR 1910.147(d)(4)(i): Lockout or tagout devices were not affixed to each energy isolating device by authorized employees: a) Repelletizer #1 (Old); On or about January 26, 2017 and at times prior thereto the employer exposed employees to potential hazardous energies in that employees were required to work on the Repelletizer to replace blades on the cutter without first affixing a lockout device on the machines isolating device to secure the machine from the energy source.
