29 CFR 1910.147(d)(5)(i): Following the application of lockout or tagout devices to energy isolating devices, all potentially hazardous stored or residual energy were not relieved, disconnected, restrained, and otherwise rendered safe: (a) On or about November 14, 2016, the employer did not assure that all potentially hazardous stored or residual energy sources such as pneumatic and electric on foil press machines are relieved, disconnected, restrained, and otherwise rendered safe, exposing foil press operators and mechanics to hazards associated with the unexpected release of energy; (b) On or about February 15, 2017, the employer did not assure that all potentially hazardous stored or residual energy sources such as gravity on Franklin foil number machines (numerators) are relieved, disconnected, restrained, and otherwise rendered safe, exposing mechanics to hazards associated with the unexpected release of energy.
