29 CFR 1926.1204(e)(1)(ii): The permit confined space was not tested to determine if acceptable entry conditions existed before authorized entry was begun, if entry is authorized, continuously monitor entry conditions in the areas where authorized entrants are working, except that employers may use periodic monitoring in accordance with paragraph (e)(2) of this section for monitoring an atmospheric hazard if they can demonstrate that equipment for continuously monitoring that hazard is not commercially available: a. E E Robinson Park (Level Creek), 800 Peachtree Industrial Blvd. Sugar Hill, GA - An employee was exposed to asphyxiation hazard. The employer did not continuously monitor the permit space to verify that atmospheric conditions remain acceptable during entry. The employer removed the manhole cover and purchased a flash light with the expectation that the employee would break the plain, on or about September 6, 2016.
