29 CFR 1910.305(g)(2)(ii): Flexible cords were not used in continuous lengths without splice or tap: (a) On or about April 3, 2015, at the above addressed worksite, the Fill-Rite pumps on two 1000 gallon diesel storage tanks were wired with spliced temporary wiring and plugged into a wall receptacle, exposing the employees to the hazards of electrical shock/burns and/or fire and explosions. (b) On or about April 3, 2015, at the above addressed worksite, the NexGen Coriolis Flow Transmitter in the biodiesel storage area was wired with spliced temporary wiring and plugged into a multiple outlet strip then further plugged into a wall receptacle, exposing employees to the hazards of electrical shock/burns and/or fire and explosions. (c) On or about April 3, 2015, at the above addressed worksite, the ChemTech PulsaFeeder pump on top of the BioExtend tote in the biodiesel storage area was wired with spliced temporary wiring, run in series with another flow switch and then plugged into a wall receptacle, exposing employees to the hazards of electrical shock/burns and/or fire and explosions. (d) On or about April 3, 2015, at the above addressed worksite, the McDonnell & Miller Flow Switch located in the biodiesel storage area was wired with spliced temporary wiring, run in series with the ChemTech PulsaFeeder and then plugged into a wall receptacle, exposing employees to the hazards of electrical shock/burns and/or fire and explosions.
