29 CFR 1910.147(d)(4)(i): Lockout or tagout devices were not affixed to each energy isolating device by authorized employees: a) Production Area- The employer failed to ensure that persons who performed set-up, adjusting, and servicing on Presses B1-22, TP-110, TP-60, 45-T, NC-150, and their auxiliary equipment, such as feeders and straighteners, affixed energy isolating devices to each energy source. b) Secondary Operations Area- The employer failed to ensure that persons who performed set-up, adjusting, and servicing on the five Air Presses, affixed energy isolating devices to each energy source. In accordance with 29 CFR 1903.19(d), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET), and in addition, documentation demonstrating that abatement is complete must be included with your certification. This documentation may include, but is not limited to, evidence of the purchase or repair of the equipment, photographic or video evidence of abatement, or other written records.
