29 CFR 1910.134(k)(1)(iv): The employer did not ensure that each user could demonstrate knowledge of how to inspect, put on, remove, use, and check the seals of the respirator: a) Establishment - Sprayfoam insulation applicators and technicians who wore tight fitting respirators including, but not limited to 3M negative pressure half mask and full face air purifying respirators, and supplied air respirators when applying insulation containing 4,4 Diphenylmethane diisocyanate (MDI), Diphenylmethane diisocyanate (2,2,2,4), and Polymethylene polyphenyl were not trained per the requirements of the standard. On or about 1/6/15. NOTE: THE EMPLOYER IS REQUIRED TO SUBMIT ABATEMENT CERTIFICATION FOR THIS ITEM. FAILURE TO COMPLY WILL RESULT IN AN ADDITIONAL PENALTY OF $1000.00 IN ACCORDANCE WITH 29 CFR 1903.19.
