29 CFR 1910.119(j)(2): The employer did not establish and implement written procedures to maintain the on-going integrity of process equipment. a. Toledo Refining Company LLC located in Oregon, Ohio: On or about September 2, 2014, the employer did not ensure the mechanical integrity program was being implemented to maintain the ongoing integrity of process heater H-6104 located in Plant 6. The convection section of the process heater was initially scheduled to be replaced in 2013 but was later rescheduled to be replaced in Fall of 2014 without evaluating the integrity of the equipment.
