29 CFR 1910.147(c)(7)(i)(A): Authorized employee(s) did not receive training in the recognition of applicable hazardous energy sources, the type and magnitude of the energy available in the workplace, and the methods and means necessary for energy isolation. On or about August 27, 2014 at 2345 Murphy Blvd., employees perform servicing and maintenance activities on equipment including, but not limited to, set up and tools changes, did not receive training on the type and magnitude of the energy available in the workplace and the methods and means necessary for energy isolation. a. DP-2, employees working on the DP-2 are exposed to an amputation hazard. The employees making the tool changes that required LOTO only turned off the machine. Employees were not trained as authorized employees when performing set up and tool changes. b. DP-4 (Davenport), employees working on the DP-2 is exposed to an amputation hazard. The employees making the tool changes that required LOTO only turned off the machine. Employees were not trained as authorized employees when performing set up and tool changes. c. P-12 (Big Green), employees working on the DP-2 is exposed to an amputation hazard. The employees making the tool changes that required LOTO only turned off the machine.
