29 CFR 1910.178(m)(2): Personnel were allowed to sit on the elevated loaded portion of a powered industrial truck. (a) On August 20, 2014, at the facility located at 5801 E Front St, Kansas city, MO., an employee was seriously injured while riding on an elevated loaded powered industrial truck in order to balance the load during transport and off loading. As a result, the employee received minor struck-by injuries after falling off and then being struck by the falling load. Abatement certification is required for this item; use the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET for this purpose. ( Proof of training to address this issue)
