29 CFR 1910.147(f)(3): 29 CFR 1910.147(f)(3): When servicing and/or maintenance was performed by a crew, craft, department or other group, the employer did not ensure that employee(s) utilized a procedure which affords the employees a level of protection equivalent to that provided by the implementation of a personal lockout or tagout device: (a) On or about June 16, 2014 the inspection revealed that during crew maintenance the employer did not ensure that employees use a group lockout system requiring each exposed employee to use a personal lockout device. Located throughout the facility at 1777 NW 44th Street in Lincoln City, Oregon.
