29 CFR 1910.119(l)(1): The employer did not implement procedures to manage changes to process chemicals, technology, equipment, and procedures, and changes to facilities that affect a covered process. On or about 6/10/14, the employer did not implement procedures to manage changes to the ammonia refrigeration system at the facility when new equipment/processes were installed as a part of an ongoing MOC at the facility at the time of their implementation. Equipment includes: 1 High Pressure Receiver (York #163578) placed on line 5/26/2014 2 Recirculator (Frick #115067) placed on line 6/9/2014 In accordance with 29 CFR 1903.19(d), abatement certification is required for this violation (using the CERTIFICATION OF ACTION WORKSHEET), and in addition, documentation demonstrating that abatement is complete must be included with your certification. This documentation may include, but is not limited to, evidence of the purchase or repair of the equipment, photographic or video evidence of abatement, or other written records.
