29 CFR 1926.1060(a)(1)(iii): The employer did not ensure that each employee had been trained by a competent person in the proper construction, use, placement, and care in handling of all stairways and ladders: Employees who did not received ladder safety training did not recognized the fall hazard related to utilizing a portable ladder to access/egress a roof work surface without the side raills extending three (3) feet above the upper landing surface.
