29 CFR 1910.147(c)(7)(iii)(A): Retraining was not provided for authorized and affected employees when there was a change in their job assignments, a change in machines, equipment or processes that presented a new hazard, or when there was a change in the energy control procedures: Employees performing lock out during regular servicing of the Harris (HSC) shear were not retrained on potential hazards when the equipment was modified: a.) I-beams were used in lieu of safety pins to block the crosshead; b.) The liners were bent and crumpled in the clamp slide, and bolts were replaced with welded components. c.) Employees were not separately instructed on which piece of equipment needed a particular block. d.) Employees were not trained in the possible hazards of liner modification or removal.
