29 CFR 1926.1408(d)(1): Part(s) of the equipment, load line, or load (including rigging and lifting accessories) were allowed below an energized power line when the exceptions to paragraph (d)(2) did not apply. On or about 21 April 2014, and at times prior thereto, a rough terrain crane with a boom arm was allowed to operate under an energized power line within 10 feet, exposing the operator and co-workers nearby to an electrical hazard of approximately 25 kV 3-phase voltage.
