29 CFR 1926.502(i)(3): All floor hole covers were not secured when installed so as to prevent accidental displacement by wind, equipment, or employees: a) A plywood floor hole cover on the 3fl was not secured during concrete construction; employees can fall 10 feet or step into the 2 ft wide hole if the cover shifts. The cover was cut to the exact size as the floor hole so can collapse if displaced (shifted) one inch. Location: 81 Fleet Place, Brooklyn, N.Y; construction worksite. On or about 2/24/2014 NOTE: THE EMPLOYER IS REQUIRED TO SUBMIT ABATEMENT CERTIFICATION FOR THIS ITEM IN ACCORDANCE WITH 29 CFR 1903.19.
