29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks: a) Maintenance Shop  on or about February 05, 2014, the DoAll band saw did not have the unused portion of the blade guarded, exposing employees to an amputation hazard. b) Maintenance Shop  on or about February 05, 2014, the Cincinnati Gilbert Milling Machine was not provided with a guard to cover the rotating drill, exposing employees to hazards of the rotating chuck and flying chips. c) Maintenance Shop  on or about February 05, 2014, the Lagun Milling Machine was not provided with a guard to cover the rotating drill, exposing employees to hazards of the rotating chuck and flying chips. d) Maintenance Shop  on or about February 05, 2014, the Cincinnati Milacron Milling Machine was not provided with a guard to cover the rotating drill, exposing employees to hazards of the rotating chuck and flying chips. e) Maintenance Shop  on or about February 05, 2014, the Cincinnati Milling Machine was not provided with a guard to cover the rotating drill, exposing employees to hazards of the rotating chuck and flying chips.
