29 CFR 1926.451(g)(4)(iv): Midrails were not installed at a height approximately midway between the top edge of the guardrail system and the platform surface: a) Wine on the 9, 6531 U.S. 9, Howell, NJ 07731: On or about May 10, 2013, an employee was exposed to fall hazards of approximately 13.5ft while working from a scaffold without adequate guardrails. There was no midrail installed. NOTE: IN ADDITION TO ABATEMENT CERTIFICATION, THE EMPLOYER IS REQUIRED TO SUBMIT ABATEMENT DOCUMENTATION FOR THIS ITEM. FAILURE TO COMPLY WILL RESULT IN AN ADDITIONAL PENALTY OF $1000.00 IN ACCORDANCE WITH 29 CFR 1903.19.
