29 CFR 1910.119(e)(6): The employer did not update and revalidate the process hazard analysis with a team meeting the requirements in paragraph (e)(4) of this section: a) Old Acid Room Area - The employer did not revalidate the PHAs associated with the Formaldehyde Loading System using a team that met the requirements of paragraph (e)(4) in that the team lacked a member with experience and knowledge specific to the process being evaluated, on or about September 26, 2012. Abatement certification required within 10 days after abatement date. The certification shall include a statement that abatement is complete, date and method of abatement, and states employees and their representatives were informed of this abatement.
