29 CFR 1910.1030(d)(3)(iii): The employer did not ensure that appropriate personal protective equipment in the appropriate sizes was readily accessible at the worksite or issued to employees: a) On or about June 27, 2012 and at times prior, Key Energy Services Inc. did not ensure that employees were protected from blood borne pathogen hazards. During the response, to an employee who fell, responders did not have access to appropriately sized personal protective equipment.
