29 CFR 1910.178(l)(4)(iii): An evaluation of each powered industrial truck operator's performance was not conducted at least once every 3 years. (a) On June 23rd, 2012 it was determined that employees of RJD Group were operating powered industrial trucks without having any refresher training in the last three years exposing employees to struck by and caught under hazards associated with forklifts.
