1910.119(e)(3)(iii)* The process hazard analysis shall address: Engineering and administrative controls applicable to the hazards and their interrelationships such as appropriate application of detection methodologies to provide early warning of releases: The employer's process hazard analyses for the refinery including, but not limited to the previous PHA's for the #5 Crude Unit did not identify the specific engineering and administrative controls applicable to the lighting of H-501. Employees were exposed to the process related hazards associated with the fire and explosion that occurred on May 25, 2010, due to the lack of specific identification of engineering and administrative controls and their interrelationships associated with the lighting procedures of heater H-501 after a turnaround.
