29 CFR 1926.404(f)(6): The path to ground from circuits, equipment, or enclosures was not permanent and continuous: a) On or about March 17, 2009 and at times prior Petrochem Insulation Inc. did not ensure that employees were protected from electrical hazards. An electric extension cord used to power a hand shear did not have a ground prong.
